Social Worker (Crisis & Transfer Program Coordinator)
The Crisis and Transfer Coordinator (CTC) is a Social Work Coordinator who is administratively and clinically responsible for the Crisis and Transfer Program at Cheyenne VAHCS. Primary responsibilities will be to serve as the VA's Crisis Coordinator functioning as the SPED (Safety Planning in the Emergency Department) lead consultant and educator. Serves as SPED (Safety Planning in the Emergency Department) Lead Consultant and Educator. Collaborates with the Suicide Prevention team to monitor high risk Veterans and ensure engagement with mental health providers. Leads the Cheyenne VA Crisis Team, tracks SPED dashboard, identifying areas needing improvement and reports to MEB as requested.
