Confidential Search: Vice President/Senior Vice President of Ambulatory Care Management

A major integrated health system and a national population health innovator have launched a transformative joint venture focused on delivering a highly coordinated, value-based ambulatory care experience across a multi-state footprint. This initiative centers on accelerating value-based care and strengthening the care management infrastructure that supports proactive, patient-centered, population-based ambulatory care. Together, we are reimagining how comprehensive, longitudinal care is coordinated and delivered across one of the nation’s leading integrated health systems. 

We are seeking a Vice President or Senior Vice President of Ambulatory Care Management to lead enterprise-level care-management strategy, population health integration, and care-model transformation. This leader will work in a dyad relationship with the Health System’s leader of population health and will be accountable for building a seamless, integrated ambulatory care-management ecosystem that advances value-based performance, enhances proactive outreach and coordination, strengthens provider and care-team alignment, and delivers measurable improvements in quality, outcomes, and total cost of care. 

This is a unique opportunity for an accomplished ambulatory or population-health executive to operate at the intersection of care management, value-based care, clinical transformation, and data-driven population health strategy, shaping the future of coordinated ambulatory care across a large, diverse patient population. 

Key Focus Areas 

• Lead strategy and operations for a large ambulatory enterprise 

• Integrate population health, care management, analytics, and virtual care 

• Improve access, patient experience, provider engagement, and value-based performance 

• Drive operational excellence, standardization, and financial stewardship 

What We’re Looking For 

• 7+ years in ambulatory operations within a complex health system; experience with a health plan would be a plus 

• Experience in value-based care, population health, or care-model redesign 

• Strength in building collaborative relationships across multiple stakeholders within a health system and the ability to foster alignment between the health system and its strategic partner 

• Master’s degree preferred
