SummaryThe CPS will be assigned to Patient Aligned Care Teams (PACT) in the primary care clinics. Teams are located in the main hospital and associated community based outlying clinics. The CPS functions at the highest level of clinical pharmacy practice to provide evidence-based pharmaceutical care to patients.
