The Suicide Prevention Program (SPP) is recruiting for a Suicide Prevention Case Manager (SPCM) Social Worker. The incumbent will have the responsibility to coordinate and monitor services for Veterans designated as high risk for suicide and act as the transitional coordinator of care for these patients. The social worker is responsible for coordinating the care of high-risk patients throughout the Syracuse VA medical center, including Community Based Outpatient Clinic's and community agencies. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
