LOCATION: BAYLOR SCOTT & WHITE COLLEGE STATION , TXDEPT: TELEMETRYSCHEDULE: FULL TIME DAYS JOB SUMMARYThe Nurse Navigator is a Registered Nurse who facilitates patient (s) continuity of care with the healthcare team in an efficient and timely manner. The RN acts in an intake model, assesses the patient needs, assist with planning, organizing, implementing and evaluating the patient care needs with the healthcare team. The Nurse Navigator functions autonomously and in collaboration with other members of the healthcare team as a patient advocate, educator and coordinator. The Nurse Navigator possesses strong verbal and written communication skills.
