Job SummaryCoordinates patient care and maximizes patient satisfaction and experience. Partners with care providers to serve as a liaison to ensure effective communication with patients.Job DutiesProvides discharge information.Partners with providers to ensure a smooth transition at discharge to home or other facilities as required.Ensures additional resources are made available for the patient as needed.Effectively collaborates with other healthcare providers and non-Geisinger employees in coordinating and implementing the patients plan of care.Maintains systems and processes to assure timely follow-up and surveillance of a defined group of patients.Serves as liaison with clinical and administrative personnel.
