SummaryThe position is assigned to Outpatient Nursing Service at the Mann-Grandstaff VA Medical Center and is located at Coeur d' Alene Community Based Outpatient Clinic (CBOC) in Coeur d' Alene - Idaho. The PACT LPN is responsible for observing - recording and reporting changes in the patient's condition - as well as assisting in the development and revision of the patient-driven holistic care plan for life.
