Case Manager/Social Worker, MSW - Patient Case Management
OverviewThe Social Worker is assigned to specific clinical care units and/or specific patient populations to provide services to patients and families whose medical, psychological and/or social situation impacts compliance with the physician care plan, the provision of appropriate health care services for the patient, and/or timely discharge. Coordinates the assistance necessary to meet patient specific needs relative to placement, basic living resources, home health resources, therapeutic counseling, and post-hospital support in order to optimize outcomes.
