Orthopedic Nurse Navigator, Days
Job DescriptionAssessment and development of an individualized plan of care through initial and periodic assessments and through communication with patient/family, physicians and other staff. Identifies physical, psychosocial, spiritual and social needs and coordinates the development and implementation of a plan of care to meet those needs effectively and efficiently.Maintain close communication with the Medical Director Joint Replacement Center (JRC) to evaluate all barriers to care, including medical, social and economic.The Nurse Navigator will be an integral part of the planning process from the point that a patient is identified as needing a total joint throughout the recovery period.
