SUMMARY OF POSITION:
The RN/Registered Nurse is responsible for coordinating the timely, efficient and effective movement of patients into, through and out of patient care systems. The RN also serves in management and supervisory capacities as a role model, manager, consultant, collaborator, educator, and clinician.
Field phone calls from patients and families on inpatient service, provide triage, education and supportive counseling, collaborating with primary teams to ensure clear communication
Provide education on safe opioid management, bowel management, red flags and signs of when to call for hel.
Provide low acuity follow-up visits
Anticipate needs for prior authorization of symptom management medications and facilitate approval prior to discharge.
Initiate follow-up calls of high risk patients after hospital discharge
Coordinate with outpatient RN coordinator to ensure follow-up and seamless symptom management
Provide education to rotating learners, staff, community on palliative care principals
Participate in team QI and data collection
License to practice as a registered nurse in the state of Pennsylvania required.
Bachelor's degree in nursing required.
Three (3) years of nursing experience required.
