M Health Fairview has an immedaite opening for an RN Care Coordinator at the M Health Fairview Specialty Clinic located in Edina, MN.
This role will primaly be working in general surgery-breast care.
This is a 1.0 FTE (80 hours per two week pay period), benefits-eligible opening.
The primary function as the role of Cancer Coordinator for patient population that has receiveddiagnostic confirmation of breast cancer. Ability to work with patients in an environment with time restraints. Need to be familiar with treatment options from current literature standards. Ability to anticipate patient needs and take appropriate course of action using approved guidelines. Excellent problem solving skills. Clinical coordination of care across the care continuum including: Radiation Oncology, Medical Oncology, Plastic Surgery, Brest Surgery, Genetics and Imaging. Assistingpatient navigation through process including follow up and resource support. Assist in providing communication and coordination with PCP. In partnership with multidisciplinaryteam, develop database/registry and participatein cases submitted to Cancer Conference.
Coordinates care for patients and families in collaborationwith multidisciplinaryteam across the continuum of care with an emphasis on newly diagnosed in patients
Synthesizes assessment data and resulting diagnosis. Plans interventions and expected outcomes to provide patient and family necessary information, Communicates with referring health care providers regarding comprehensive plan of care for patients and family.
Assists the patient in coordinating multiple specialty appointments and provides education for patients
Coordinates multidisciplinaryteams conferenced
Works closely with physicians and departments to deliver coordinated care in a timelymanner
Works with multidisciplinary team to develop quality monitors to measure patients outcomes and programperformance
Collaborates with Mammotrak and Cancer Data services on data base management
Assesses learning needs of patient, family, and health care team: creates, implements and evaluated multidisciplinaryeducational plan
Utilizes current research, scientific literature, and advanced practice knowledge when providingeducationfor patients, families, community, and staff
consults with and teaches patients and families about their diagnosis, diseaseprocess and plan of care. Collaborates with surgeons, medical and radiation oncology regarding specificteaching needs
Provides formal and informal education for health care and hospitalstaff as needed
Serves as a resourcefor the community to provide public education
Participates in ongoing education activitiesto expand advanced clinical knowledge and enhance role performance.
In collaboration with physicians and provides consultationsto infulancethe comprehensive plan of care for patients, family, communityand program areas
Serves as clinical experts/resource to oncology patients, families, health care team and community
Provides phone consultations, advocates for care or referrals to patients, families, other health care professionals in the community
Serves ad Program Liaisonto community through a variety of communication sources
Assess, monitor and assist patients from diagnosis on and identified expectednursing outcomes
Documents data and updates the plan of care
Provides dischargeinstructions to patients and families, post procedure
