The Palliative Care and Hospice Nurse Practitioner is a registered nurse with advanced clinical preparation for directing and delivering of comprehensive palliative and hospice care to Veterans with life threatening conditions. The Palliative Care and Hospice APRN is a privileged licensed independent provider. The Palliative Care and Hospice APRN reports to the Associate Chief Nurse, Extend Care and Rehabilitation Services and is a member of the Palliative and Hospice interdisciplinary team. Major duties will include, but are not limited to: Active member of the Palliative Care and Hospice interdisciplinary team in the care of Veterans with a wide range of medical conditions and terminal diagnosis. Provides emotional support to Veterans and families facing devastating diagnoses Completing comprehensive medical history and physical examination. Writing orders, diagnostic tests, treatments, and consults. Interpreting test results, adjusting testing regimens and consults with other health care providers. Completing consultation for inpatients in all levels of care and outpatients. Educating staff to meet changing program and service needs. Functions in and administrative and clinical capacity, and as an independent clinical capacity providing direct patient care. Assessing, diagnosing, planning, and proscribing pharmacologic and non-pharmacologic treatment including mind, body and spiritual interventions. . Assess and treat symptoms to promote maximum level of wellness and independence for the patient with terminal illness Develops treatment plans to relieve pain, nausea and other symptoms to reduce pain and distress.
