This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more The Facility Telehealth Coordinator/Nurse Manager manages care across the health care continuum. He/she works closely with primary care provider(s) and other health care professionals and team members, other clinics, internal services and community agencies. The incumbent holds a facility level position, reporting to the Associate Chief Nurse of Telehealth and Case Management, with coordination and input from the VISN Telehealth Manager. The incumbent is responsible for overall planning, coordination, implementation, management, and evaluation of clinical applications of telehealth within the facility organization, with particular emphasis on improving access to Veterans identified as "rural" and "highly rural". The incumbent works closely with network and facility leaders and staff to incorporate new concepts and programs into extant systems and provides education and training to all facility staff impacted by program, role, or technology changes. Matrix lines of authority ensure that VISN 1 and VHA goals and strategic directions are fully supported by telehealth programming through collaborative efforts with facility and network leaders. The incumbent is knowledgeable of overall facility and network goals, missions and priorities to ensure that the telehealth programming does not impact adversely upon other programs or resources. He/she is knowledgeable of funding sources and methodologies including Office of Rural Health, the area Rural Health Resource Center, VERA, third party collections, sharing agreement revenues, contracts and enhanced use revenue. General: Cooperating with the VISN Rural Health Planner and Telehealth Manager, incumbent engages in outreach planning and activities, including work with potential non-VA community partners in an effort to improve access to rural and highly rural veterans to healthcare services via telehealth initiatives and programming. Establishes guiding processes, principles and policies for the development and implementation of patient and family centric telehealth programs at the facility, and for the operation, management and oversight of Telehealth Programs. Ensures functional linkages across the telehealth continuum, (i.e. HT, CVT, and SF) with regard to the various telehealth clinical applications, including but not limited to telemental health, teleretinal imaging, teledermatology, telesurgery, home telehealth, telepharmacy, telerehabilitation, telepatient education, and teleendocrinology. Program Development: The incumbent develops specific telehealth applications in collaboration with VISN and station telehealth oversight councils, clinical, administrative, and technical staff as needed. Develops policies and protocols for delivery of telehealth applications. Develops tracks and disseminates, as appropriate, outcome measures, including clinical data, patient and travel costs, and patient and staff satisfaction measures. Program Management/Administration: Develops, implements, and maintains processes and mechanisms for scheduling telehealth clinic resources. Ensures that all telehealth clinics are authorized, use proper DSS coding, and are named in compliance with Network clinic telehealth naming conventions. The incumbent develops mechanisms to evaluate and monitor the quality, safety, efficiency, utilization, productivity, and effectiveness of telehealth. He/she will direct the identification and incorporation of computerized data ad information management tools and systems needed for telehealth clinical care, as well as management of workload and budget. Consistently monitors utilization, fiscal impact, challenges, and outcomes of Telehealth applications, shares and disseminates information for continual quality improvement. Seeks opportunities for appropriate program expansion, including external support and resources where indicated. Develops and proposes an annual telehealth program budget in coordination with facility leadership and the VISN Telehealth Program office. Technology The incumbent provides leadership in evaluation, implementation and management of all clinical telehealth technologies. Communicates and collaborates with VISN Telehealth Technology Advisory Group, the Office of Information and Technology and other technical staff in the development, implementation and operation of telehealth applications with regard to technology and connectivity, which includes wide area network (WAN); local area network (LAN); and bandwidth issues.
