The Registered Nurse will function as the Patient Aligned Care Team Nurse in Temple, TX. You will execute position responsibilities that demonstrates leadership, experience, and creative approaches to management of complex patient care. You will use the nursing process and evidence-based practice to collaborate with the Teamlet (Patient, Primary Care Provider, RN Care Manager, Clinical Associate, and Clerical Associate) and expanded Team (family/caregiver, internal, and community -based services involved in providing care to the patient) in developing the patient driven care plan for life. The PACT RN is a member of the Teamlet team. You are responsible for collaborating with services internal and external to the VA to facilitate care transition to effectively meet the patients' needs. Provide patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's personalized health plan/goals. Demonstrate leadership by serving as an advocate for patients, and a team leader with colleagues as you continue to enhance your own and the team's professional growth, development, and practice. Possess the knowledge and skills to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting to facilitate the effective care of patients. Within the framework of the nursing process and through highly developed clinical expertise, the RN can assess, monitor, and treat patient responses to life threatening health problems. Responsible for collaboration with administrative and clinical staff in actively managing and planning for actual and potential needs of patient population groups.
