As a member of the LightBridge executive leadership team, the Associate Director of Patient Care Services is responsible for the implementation, supervision and oversight of all patient care programs as well as achievement of both clinical and administrative outcomes. This position will focus on the overall clinical management of the hospice and palliative programs of LightBridge, including quality assurance, customer service and daily operations.
Key responsibilities and functions:
Assure coordination of services 24 hours a day, 7 days a week
Manage team schedule and territory assignments to assure a balanced workload
Interview, select, train, supervise and evaluate clinical staff in collaboration with other team managers and the Director of Patient Care Services
Assist in monitoring the type and level of services provided to assure they are appropriate, cost effective and in line with patient/family needs
Serve as a resource and mentor for staff regarding clinical issues, documentation, team problem solving and customer service
Participate in team meetings, providing input for the update and revision of plans of care in concert with the changing needs and expectations of the patient/family
Inspire operational excellence and ensure compliance with internal policies, Medicare guidelines and JCAHO/state regulations

Bachelor’s degree in nursing preferred

A minimum of 3 years’ recent experience in a hospice, palliative or home care, at least one year of which was in a supervisory or administrative capacity
Ability to respond to needs of patients/families/caregivers in varied settings and to function effectively in a nursing facility environment
Comfortable with computers and mobile technology (cell phone, electronic medical record)
Ability to handle multiple priorities, documentation requirements and deadlines
Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
Communicate effectively in English both orally and in writing, sufficient to prepare and send reports, interact with clients, staff and peers (bi-lingual preferred)
