This role provides clinical and nursing administration support to the Palliative Care program for all the Baptist Health System Hospitals
The position is a registered nurse who is accountable for effective coordination of patient with palliative care needs. The position systematically triages patients based upon clinical problems and educational needs, provides coordination of care for the palliative care patient, and is also a resource to clinic staff in regard to the direct provision of care.
The position establishes goals and strategies for meeting the continuing care needs of the patient, family, and/or other care provider.
The position will collaborate with stakeholders to assure that systems of care are in place for patients to move across the continuum of care. This includes development of patient care protocols, order sets, and systems that allow care to take place in an effective and efficient manner. Also puts systems in place to evaluate the effectiveness of patient care.
Required: Academic degree in nursing Preferred: Bachelors or Masters degree
Required: 2 Years of Palliative Care Nursing experience in a hospital environment.
Required: Must be currently licensed, certified or registered to practice profession as required by law, regulation in state of practice or policy. CPR
Preferred: CHPN, ACHPN or other Palliative Care Nursing Certification strongly preferred
