The Clinical Pharmacist provides quality pharmaceutical care defined as being responsible for assuring the pro-vision of safe, efficacious and cost-effective medication
therapy for all patients. The Clinical Pharmacist will develop, recommend and monitor evidence based pharmacotherapy care plans for patients cared for by Geisinger
with emphasis on patient outcomes and quality of life. These care plans include the assessment of patient’s medications for indication, effectiveness, safety and
adherence issues as well as patient and laboratory parameter monitoring, documentation requirements and outcomes. The Clinical Pharmacist will utilize integrated
and routine communication with all health care professionals and with patients concerning medication therapy management. Provides outcomes and evidenced
based pharmaceutical care services to the patients in the hematology/oncology service line. Provides assistance and in-depth drug information to medical personnel;
performs operational/clinical functions or projects as assigned; assesses patient’s medications for indication, effectiveness, safety and adherence issues and orders lab
tests in the assessment process; meets the qualifications to provide care for patients in the specific age range on their assigned clinical area (inpatient unit, outpatient
infusion center, outpatient MTDM clinic); responsible for providing education and preceptorship which meets the requirements of residency training program (where
applicable) and pharmacy student programs as well as mentoring and training other pharmacists
Performs duties to provide the highest level of patient care and maximize patient satisfaction.
Works under the general direction of the Clinical Coordinator, Assistant Directors of Pharmacy (where applicable), Acute Care Pharmacy Managers (where applicable)
and the Director(s) of Pharmacy.

EDUCATION AND/OR EXPERIENCE:
Doctor of Pharmacy (PharmD) degree from an accredited college and current licensure or eligibility for licensure by the Pennsylvania State Board of Pharmacy required.
In the absence of a PharmD, a Bachelor of Science in pharmacy with appropriate clinical pharmacy experience will substitute.
Pharmacy Graduate Year 1 (PGY1) residency experience or equivalent clinical pharmacy experience required. Preference will be given to candidates with Pharmacy
Graduate Year 2 (PGY2) residency in Hematology/Oncology.
