Provides an integrated continuum of care for patients from preadmission to post-discharge.Screens for discharge planning needs and initiates a plan for community resources and/or placement in appropriate post-discharge facility.Serves on committees as requested.

Minimum Experience:1 to 2 years of job-related experience preferred, including experience as a licensed clinical social worker, experience in an acute care setting, knowledge of federal and state laws and Medicare/ Medicaid calculations, and experience with computer software.
Required Certifications/Registrations/Licenses:S.C. licensure in Social Work.
