JOB TITLE: Intake Coordinator Registered Nurse, Geisinger At Home

Screens potential new enrollees through thorough medical record review, care team interview if needed and patient/family interview. Evaluates patient findings and coordinates new enrollees to the appropriate care team within Geisinger at Home. Engages new candidates for Geisinger at Home and provides comprehensive description of the program to the patient and family. Serves as embassador for Geisinger at Home with targeted physcians and community members training and engagement.
Triages acute care calls from patients, family and clinical teams. Resolves issues that do not require Geisinger at Home’s acute team to patient/family satisfaction. Makes appropriate hand-offs to Field Coordinator team or other team members on the Home Based Medical Care, Community Palliative Care, or Mobile Integrated Health teams.
Assists care team staff with outreach activities to support care initiatives for patients throughout the care continuum as well as enrollment in hospitals and nursing homes.

Demonstrates strong interpersonal, oral and written communication skills.
Demonstrates the ability to be a self-starter with strong organizational and time management skills. Detailed oriented, extremely well organized and able to plan and problem solve.
Demonstrates ability to work independently.
EDUCATION AND/OR EXPERIENCE:
Registered Nurse with valid Pennsylvania licensure to practice.
A minimum of two years clinical experience.
Computer literacy required with specific experience in Microsoft applications and Windows operating platform.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Varies settings including but not limited to office, patients home, and hospital.
Travel required, personal vehicle with mileage reimbursement
Day shift with on-call schedule after hours.
