In this very special role, you will coordinate the outpatient care of pediatric pulmonary and cystic fibrosis patients as well as provide patient and family education. This involves collaborating with an interdisciplinary team of dedicated professionals to develop and implement a care plan, including providing detailed care instructions. You will also conduct the outpatient pulmonary clinic and home ventilator program.

We’re seeking a self-directed professional with a current California RN license and: BSN or MSN Ability to coordinate patient care using a team approach In-depth understanding of medical terminology Technical writing skills to document assessments Background in electronic data collection and analysis Ability to handle sensitive patient information ethically and confidentially Computer proficiency with Word, Excel, and PowerPoint Exceptional interpersonal and bilingual (English/Spanish) communication skills Strong organizational abilities Ability to handle case management for individual patients and their families
Preferred: Asthma educator certification Familiarity with the CCS and Medi-Cal programs Working knowledge of pediatric pulmonary and cystic fibrosis centered care CCS paneled in pediatrics (will be expected to apply for paneling upon hire)
