POSITION SUMMARY: Renders professional nursing care to patients assigned in the CancerCenter, in support of medical care as directed by medical staff and pursuant to objectives and policies of the hospital. Work encompasses direct and indirect patient care and collaboration with other disciplines involved in the provision of care. The Oncology Patient Navigator acts as the primary point of contact after initial diagnosis and connect patients with the physician who will provide their initial treatment/care. The Patient Navigator is responsible for: providing education to patients and family members; monitoring and coordinating care with additional healthcare providers, throughout the full spectrum of care, including: diagnostic imaging, treatment and survivorship or end-of-life care; coordinating the high risk cancer programs; providing resources and support services information; and providing community outreach and education regarding health and cancer care.
Required: Graduate of an accredited School of Nursing with a minimum of 2 years acute care nursing experience. Preferred: Academic degree in nursing (Bachelors or Masters degree)
OCN Certification within 18 month
Experience/knowledge in Radiation Oncology â preferred
Experience/knowledge in Radiology â preferred
Required: Must be currently licensed, certified or registered to practice profession as required by law, regulation in state of practice or policy. CPR
