Lifespan is seeking a Clinical Pharmacist, Solid Organ (Renal) Transplant to serve Rhode Island Hospital, located in Providence, Rhode Island. In this role, you will be responsible for the coordination of and/or collaboration on solid organ transplant pharmacy clinical programs including but not limited to inpatient and ambulatory care medication therapy management, pharmacotherapeutics, formulary management, specialty pharmacy services, pharmacy residency and student preceptorship, medical resident and nephrology fellowship education, and academic lectures. In collaboration with multidisciplinary care givers throughout our hospital inpatient transplant units and ambulatory transplant clinic, activities include monitor, assess, recommend changes to and optimize medication use; assist in teaching and monitoring of patients enrolled in our specialty pharmacy monitoring program, participate in the development of therapeutic drug policy and implement corrective strategies necessary to contain drug costs; monitor pharmacy clinical services through continuous quality improvement activities; and participate in the development and implementation of policies, procedures, therapy plans, and budgets.
Bachelor of Science in pharmacy degree from an accredited college of pharmacy. Advanced study in pharmacy or health care related field, preferably a Masters or Doctor of Pharmacy degree, or comparable work experience.
Minimum of five years progressively more responsible related experience or a PGY2 ASHP-accredited pharmacy residency in Solid Organ Transplantation, preferably gained in a similarly diverse and operationally complex health care environment.
Maintains current licensure as a licensed pharmacist in the State of Rhode Island.
Demonstrated knowledge and skills necessary to provide care to neonate, child, adolescent, adult and geriatric patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
Knowledge of pharmacy information systems, automated dispensing technology and personal computers including word processing, database and spreadsheet design.
