The Director is responsible for leading a dynamic team of clinical professionals in the execution of exemplary cross continuum care coordination at Bethesda Health. This Director of Care Coordination - RN works with Bethesda Health physicians, peers, community colleagues, as well as system leaders to identify opportunities to continually improve patient care services. To then execute collaborative practices that exemplify Bethesda Health’s commitment to patient centered care and community engagement. Director for Care Coordination is responsible for the planning, priority setting, coordination, implementation and evaluation of programs related to patient care coordination and throughput. Directs all aspects of the Care Coordination Model to improve the Length of Stay performance and patient throughput for Bethesda Health that will include the following; Throughput, Patient Placement, Administrative Care Management for Revenue Cycle, Care Coordination Center Operations with Leadership, and Organizational Metrics/Measures of Success. Collaborates with facility and department leaders to implement Care Coordination Model—roles, future state standard operating procedures (SOPs), Care Logistics operating system software, SNAP Huddles, and Operational Rounding. Maintains current knowledge of basic health care economics, trends and reimbursements methodologies and applies knowledge to daily practice. Director of Care Coordination - RN must have (5) five years of progressive, responsible healthcare management; strong communication and interpersonal skills with ability to work effectively, as well as collaboratively with nursing colleagues, physicians, administrators and the entire healthcare team. Florida RN licensure, and BLS from the American Heart Association. #LI-SJ1
