OverviewThe Quality and Population Health Director leads and directs the improvement of health outcomes, quality of care, patient safety, and value of care for the population of patients served by Foundation Health Partners. Develops system-wide programs and policies focusing on quality, safety, and population health. Develops and implements clinical guidelines for care designed to improve outcomes while managing costs. Tracks and presents results of improvement efforts and ongoing measures of clinical processes to management. Leads significant clinical event investigations, root cause analyses (RCAs), and peer reviews. Develops programs aimed at the prevention of disease and promotion of health. Recommends clinical initiatives, policy implications and best practices after reviewing health literature and statistics. Manages specific population health programs from planning and design through implementation and evaluation. Responsibilities1. Analyzes incidents, sentinel events, and risk outcomes with administrators, physicians, clinical leaders, and staff to identify improvement opportunities utilizing quantitative techniques and knowledge of health care operations and systems thinking. Obtains and uses literature, best practice, and benchmark data whenever possible. Manages programs for the FHP System, to include hospital, skilled nursing, and clinics.
