Meridell is seeking an exceptional Therapist who is qualified to coordinate the plan for each child's course of treatment including: admission process, modalities (specialized group therapies, if indicated), health, school, recreation, milieu, facilitate process groups, provide individual and family therapy. Is a liaison with families regarding treatment and discharge issues and is also a liaison with outside agencies and individuals including referral sources; responsible for developing and writing admission assessments, treatment plans, and discharge summaries within established time frames. Must write clear, accurate notes documenting active treatment for each therapy session within established guidelines and time frames; assume primary responsibility for discharge planning in coordination with the family and all members of the treatment team. Responsible for the assessment, design, structuring and implementation of the program services to best meet the needs of the patients and families.
